You may send your order by fax, mail or e-mail to the above addresses or contact numbers. Be sure to include product codes, billing and shipping information, quantities, and your purchase order number. If you wish to order using a credit card, we prefer that you NOT include your credit card information in the e-mail - simply indicate you wish to pay by credit card and leave a phone number, we will call you for your information. All orders that are to be paid by check require that we receive payment BEFORE a shipment can be made.
You may also download and print out the order form, you may fax OR mail this order form. If you choose to fax, DO NOT mail a duplicate as a double order may result.
